Notice of Privacy Policies

New Patient Reminders:

• Please bring ALL medication bottles with you to all visits

• Bring your insurance card with you to your first visit. 

• We will also need detailed information about the gaurantor (your insurance policy holder) such as:
  • Date of Birth
  • Social Security Number

• Bring your Drivers lisence with you to your first visit.

• Allow 3 - 5 business days for all prescription refills
